Terms & Conditions

Last updated: 6th October 2026

These Terms & Conditions apply to services provided by Sam Davies – Mind & Body.

By booking or using my services, you agree to these terms.

1. About my services

I provide:

  • Chiropractic services

  • Psychotherapy services

  • Education, lifestyle guidance and coaching-informed support where appropriate within these services

The nature and scope of care will depend on the service you are receiving and your individual circumstances.

I will discuss relevant options with you and will not provide treatment or therapy where I do not believe it is appropriate.

2. Appointments

All appointments are by appointment only. I do not operate a walk-in service.

Appointments must be booked in advance and are subject to availability.

Please arrive on time for your appointment. If you arrive late, the appointment may need to be shortened and the full appointment fee may still apply.

3. Fees and payment

Fees are communicated at the time of booking and are available on my website where applicable.

Payment is required at the time of booking to secure an appointment where this is the stated payment arrangement.

Payment can be made using the payment methods made available at the time of booking.

Where private health insurance is applicable, you are responsible for checking whether your policy covers the service and for understanding any requirements, limitations, excesses or co-payments that may apply.

4. Cancellation and missed appointments

My Cancellation Policy forms part of these Terms & Conditions.

A minimum of 48 hours' notice is required to cancel or reschedule an appointment.

Appointments cancelled or rescheduled with less than 48 hours' notice, or missed appointments, will normally result in the prepayment being forfeited, in accordance with my Cancellation Policy.

5. Your responsibility

To help me provide appropriate care, you are responsible for providing accurate and relevant information about your health, circumstances and needs.

You should tell me about any significant changes in your health or circumstances that may be relevant to your care.

You are encouraged to ask questions and tell me if you are uncomfortable with any aspect of your care or would like to discuss an alternative.

6. Chiropractic services

Before providing chiropractic treatment, I will carry out an appropriate assessment and discuss my findings and recommendations with you.

Where treatment is appropriate, I will explain relevant treatment options and obtain your consent before treatment.

Chiropractic treatment can have associated risks and side effects. These will be discussed with you where relevant.

You are not obliged to accept any treatment I recommend and can ask questions or decline treatment.

If I believe chiropractic care is not appropriate for you, or that another healthcare professional should be involved, I may recommend that you seek further assessment or care elsewhere.

7. Psychotherapy services

Psychotherapy is a collaborative process. We will discuss what you hope to gain from therapy and review your work together as therapy progresses.

Psychotherapy does not provide instant solutions, and meaningful change can take time.

I do not expect clients to commit to a predetermined number of sessions, although I may recommend an initial number of sessions where I believe this would be helpful.

I am currently practising as a trainee psychotherapist while completing my psychotherapy training. My training, supervision and professional development form part of my practice.

Psychotherapy is not an emergency or crisis service. If you are experiencing an immediate mental health crisis or are at risk of serious harm, you should seek appropriate urgent support rather than relying on your therapy appointment.

8. Confidentiality

I will treat information shared with me respectfully and confidentially, subject to the legal, ethical and professional limits of confidentiality.

For psychotherapy, the boundaries of confidentiality will be discussed as part of establishing your therapeutic agreement.

Information may need to be disclosed in circumstances such as serious safeguarding concerns, serious risk of harm, legal requirements or where disclosure is otherwise legally or ethically justified.

Further information about how personal information is collected, stored and used is provided in my Privacy Notice.

9. Children and young people

Psychotherapy: I currently provide psychotherapy to adults only.

Chiropractic: I see children from approximately seven years of age where appropriate.

For chiropractic appointments, a responsible adult must remain involved in the care of children under 16.

I will consider whether chiropractic care is appropriate for the individual child and may recommend referral or further assessment where necessary.

10. Suitability and referral

I reserve the right to decline or discontinue a service where I believe it is not appropriate for your needs, where another form of care would be more suitable, or where I do not have the appropriate expertise or resources to support you safely.

Where appropriate, I may recommend that you seek assessment, treatment or support from another suitably qualified professional.

11. Emergencies and urgent situations

Neither chiropractic nor psychotherapy appointments are intended to provide emergency medical or mental health care.

If you are experiencing a medical emergency, contact 999 or attend your nearest emergency department.

If you are experiencing an urgent mental health crisis, seek appropriate urgent support through NHS or emergency services.

12. Complaints and feedback

I welcome feedback and encourage you to raise any concerns with me as soon as possible so that I have an opportunity to understand and address them.

If you have a complaint about the service you have received, please contact me directly using the details on my website.

Psychotherapy clients will also be provided with information about the relevant complaints procedure as part of their therapeutic agreement.

13. Ending services

You are free to decide that you no longer wish to receive chiropractic care or psychotherapy.

I may also recommend ending or changing your care where I believe this is appropriate.

For psychotherapy, where possible, ending therapy will be discussed and planned collaboratively rather than ending the therapeutic relationship without appropriate consideration.

14. Changes to these Terms & Conditions

I may update these Terms & Conditions from time to time to reflect changes to my services, systems or legal and professional requirements.

The most recent version will be available on my website.
